Partners Health Plan members are rewarded, at no cost, through PHP's ChooseHealth program, just by completing two of the following goals:

How can you the benefits of the Choose Health program? The Choose Health program is for qualified PHP Care Complete FIDA-IDD Plan members. Members can contact their care team for more information.

The State of New York has created a Participant Ombudsman Program called the Independent Consumer Advocacy Network (ICAN) to provide participants free, confidential assistance on any services offered by Partners Health Plan.
